3. if the potential Medicaid applicant is already in a facility or is in good health;
4. the approximate value of all assets with the potential applicant’s name, specifically identify how much is residential real estate;
5. your goals and what you would like to accomplish.

Each case is different, so the more details you include the more information you can receive during your consultation. After you submit your question(s) you will be taken to the next page with a link to complete payment of the $25 fee.
